Integrating Clinical Pharmacists into Patient Care Team
An innovative program – led by Hae Mi Choe, PharmD – is receiving national attention, including publication in two leading Internal Medicine and Family Practice magazines. Dr. Choe is an associate dean and director of Pharmacy Innovations & Partnerships for the University of Michigan College of Pharmacy and University of Michigan Medical Group, respectively.
Recent cover story articles in Internal Medicine News and Family Practice News highlight this program designed to integrate clinical pharmacists into patient care teams that contribute to improved outcomes. The program also creates a scalable and sustainable model for clinical pharmacy services.
Patient Centered Medical Home Pharmacist Training Program
This program will prepare pharmacists to run a productive practice in an outpatient clinical environment. The training consists of 4 days of live training, including 3 half-day mock clinic sessions during which participants will interact with 18 simulated patient instructors. In the weeks after live training, several conference calls will be conducted for participants to discuss clinical or procedural issues that arise in their own clinical environments.
